If English is not the child's first language how long has it been spoken?
If so please provided details of when and where they were assessed. If you were told they had a particular type of dyslexia please provide the details. If the child has been assessed in the last 4 years please provide a copy of the assessment. Please note that if the child has not been assessed or I am not provided with a copy of the assessment I will need to ask you to fill in an additional questionnaire. If the child has an IEP and you would like to work towards these goals please provide a copy of the IEP. If you disagree with the findings of any report you send me please let me know.

Please include details of any specific programmes they are using or have used, and the extent to which they have been beneficial.
What approaches to learning are most engaging for this child? If you were teaching something new what would be most likely to stick?
Although there is much debate about whether matching learning style to input produces better learning outcomes there is agreement that teaching a child in a way they find engaging will support motivation.
What recalling methods are currently dominant? If you asked this child what happened yesterday what would they choose to do?
Please include the child's educational history if not fully outlined in the assessment report.
I can offer various levels of support to you while you follow this programme. Please indicate which will suit you best.
You can always alter the level of support at a future date.
